Telemonitoring is a disease management program that enables an individual, together with their physician and RVNA nurse, to monitor acute and chronic medical conditions from home utilizing a small electronic device that transmits information from the home to a secure website where it is read by a nurse on a daily basis.
The monitor serves as a tool for the individual patient’s use in the home environment to assist with the medical management in the disease process. The program allows clinicians to observe patient’s health care information that is sent from home using the monitor.
The monitor is customized to a patient’s individual needs in order to collect subjective and objective data such as blood pressure, heart rate, weight, blood oxygen levels, blood glucose, medication management and other medical information unique to the patient.

- Prior to receiving the monitor your RVNA manager will review your medical record and health care needs to properly program your telehealth monitor for your use.
- Specially trained staff will be monitoring your health care data and communicating with you as needed to help meet your health care needs.
The TeleHealth program is designed to assist any patient population that requires frequent physical and or medical monitoring and improvement of health status through questionaires and education.
There is research to support that management of certain chronic disease states including diabetes mellitus, congestive heart failure, chronic obstructive pulmonary disease and renal disease can be improved using home telehealth.
